Samuel Phillips LNHA

Administrator

Samuel has a Bachelor of Arts Degree in Sociology from the State University of New York at Albany; a Master of Divinity Degree from Drew Theological Seminary; and a Master of Social Work Degree from Hunter College. He is devoted to caring for the residents, cherishing their company, and being inspired by their stories.

Lizzana Mingo

Director of Social Work

Lizzana earned a Master’s Degree from Adelphi University. She enjoys working with the entire team, as each member brings a unique skill set that comes together to create a circle of care for each resident. She meets with the residents daily to ensure that all their needs are met.

Juliana Ninvalle RN, BSN, MSN, GDCN, IP-BC, WCC

Director of Nursing

Juliana has a Bachelor of Science in Nursing as well as a Master of Science in Nursing. She is committed to building a nursing team comprised of highly skilled individuals capable of caring for the most clinically complex patients and providing the highest quality of care to all.

Krisha Aragones PT, CI

Director of Rehabilitation

Krisha Aragones is the Director of Rehabilitation at Concord Nursing and Rehabilitation, bringing over a decade of dedicated experience in physical therapy and geriatric care. She earned her Bachelor of Science in Physical Therapy in 2010 and has since committed her career to enhancing the quality of life for older adults in skilled nursing settings.
Known for her strong leadership, compassion, and hands-on approach, Krisha fosters a supportive and collaborative environment among her therapy team. Her leadership style is rooted in empathy, communication, and a commitment to continuous improvement—ensuring that patient care is always at the highest standard. She is passionate about evidence-based rehabilitation and takes pride in mentoring staff, streamlining therapy programs, and advocating for each resident’s individual needs and goals.

Our team also works hard to keep therapies as consistent as possible, tracking all communications through an advanced electronic medical record system. This ensures immediate transmission of information across the entire care network.
