Eitan Nat LNHA


Eitan is one of the “bright stars” in the Concord family. He learned the job of being an administrator from one of our best, and one meeting with him will convince you that he learned it well. And that meeting will most likely take place in the hall because you won’t find Eitan sitting in his office all day. “You can’t run a skilled nursing facility or a rehab center from behind a desk. You need to be out in the facility, talking to the residents and their family members, coaching the staff, seeing what needs to be done and getting it done. That’s how you keep your finger on the pulse of the home.” With an approach like that, it’s easy to see why we matched Eitan to Concord Center. We think they’re a perfect fit. Samuel Phillips

Assistant Administrator

Samuel has a Bachelor of Arts Degree in Sociology from the State University of New York at Albany; a Master of Divinity Degree from Drew Theological Seminary; and a Master of Social Work Degree from Hunter College. He is devoted to caring for the residents, cherishing their company and inspired by their stories.

Julianan Ninvalle

Director of Nursing

Julianan has a Bachelor of Science in Nursing as well as a Master of Science in Nursing. She is committed to building a nursing team comprised of highly skilled individuals capable of caring for the most clinically complex patients and providing the highest quality of care to all.

Lizzana Mingo

Director of Social Work

Lizzana earned a Master’s Degree from Adelphi University. She enjoys working with the entire team, as each member brings a unique skill set that come together to create a circle of care for each resident. She meets with the residents daily to ensure that all their needs are met.

Cheryl David

Food Service Director

A Certified Dietary Manager, Cheryl interacts with residents in many different ways, whether it is to find out their food preferences, talk to them about their family, or simply to get them a glass of water. She is proud of her team and their commitment to the Center.

Aracelis Abreu

Director of Recreation

Aracelis attended La Guardia Community College and Brooklyn College and is Activity Director Certified by NCCAP. She is a native of Brooklyn, as are many of the residents, and she enjoys talking to them and their families to learn about their lives and the activities they prefer.

Judy Manalili

Director of Rehabilitation

Judy earned a Bachelor’s Degree in Physical Therapy from Fatima Medical Science Foundation in the Philippines and completed her Doctorate in Physical Therapy at Stony Brook University. While sharing her expertise and experience, she also recognizes that the residents also deserve love, compassion and respect.

Evelyn Vasquez

Director of Finance 

Evelyn attended Stony Brook University and is currently continuing her education at St. Paul School of Nursing. She has a passion for taking care of people in need. She started volunteering in a hospital at age 15 and has been in healthcare ever since.

For the absolute highest quality of care, Concord Nursing and Rehabilitation Center staffs medical providers on each unit who stay in constant communication with leadership throughout the day. Team collaboration is our forte, and this commitment helps us maintain an unbeatable continuum of care.

Our team also works hard to keep therapies as consistent as possible, tracking all communications through an advanced electronic medical record system. This ensures immediate transmission of information across the entire care network.
